When creating a great experience for your clients as a wedding photographer, questionnaires are such a great way to get the information you are seeking from your client, and KEEP the information that you client gives you.
When I meet my couples at a consultation, they may tell me all about their proposal or how they met, and I try and remember as much as I can. But having an engagement session questionnaire they fill out as we start talk about scheduling their session creates a document I have about their story. I also refer back to these questionnaires before our session/wedding day as well as when I blog their photos.
